Transition from Nursing Homes

The Money Follows the Person (MFP) program assists individuals transition from nursing facilities or habilitation centers to the community.  Individuals desiring to move into the community will receive help finding housing, applying for community supports and setting up their new household.  Services for Independent Living serves as a transition coordinator for this program.
