Notifying the Public of Rights under Title VI/ADA 

 

Services for Independent Living (SIL) posts a public notice of Title VI/ADA obligations can be found on our center’s website at: www.silcolumbia.org/public-notice

SIL also posts in public areas of our agency, in our board manual, employment manual, program services manual, and in our transit vehicles. 

​

SIL operates its programs and services without regard to race, color, or national origin, in accordance with Title VI of the Civil Rights Act of 1964. 

​

SIL operates its programs and services without discrimination against individuals with disabilities, in accordance with the Americans with Disabilities Act of 1990. 

​

If you believe you have been discriminated against on the basis of race, color, or national origin by SIL, you may file a Title VI complaint by completing, signing, and submitting the agency’s Title VI Complaint form.

How to file a Title VI/ADA complaint with Services for Independent Living: 

 

1. To obtain a Title VI complaint form, contact SIL Executive Director at  573-874-1646; or visit our administrative office at 1401 Hathman Place, Columbia, MO 65201. 

​

2. In addition to utilizing the civil rights complaint process through Services for Independent Living, complaints may also be filed directly with the Federal Transit Administration (FTA), Office of Civil Rights, 901 Locust St  # 404, Kansas City, MO 64106. 

 

3. Complaints must be filed within 180 days from the date of the alleged incident and should contain as much detailed information about the alleged discrimination as possible. 

 

4. The form must be signed and dated and include your contact information.
